A case manager at a comprehensive high school carries 58 students. It is late February. A goal has to be measurable in a way that survives scrutiny: a baseline, a condition, a behaviour, a criterion, and a measurement schedule. Every IEP system ships a goal bank, and every goal bank produces goals that read as measurable and are not, because the baseline came from a memory and the criterion was chosen because it was the default.

The real failure appears at progress reporting. IDEA requires that parents be informed of progress toward annual goals at least as often as parents of children without disabilities receive report cards. In practice a case manager writes progress narratives for 58 students in one evening at quarter end, from whatever data exists, which is usually not the data the goal specified. When a hearing officer later reads that goal and the progress reports side by side, the mismatch is the finding.

The fix is not better wording. It is connecting the goal to a data collection method at the moment the goal is written. If a goal says four out of five trials across three consecutive sessions, the system should create the trial collection instrument, put it in front of the provider on their schedule, and build the progress report from the collected data automatically. Design for a phone in a hallway with one hand free and the collection happens. Then the progress report writes itself, the goal and the evidence match, and the quarter end evening disappears.

The IEP is a promise of specific service minutes in a specific setting delivered by a specific type of provider. The district's ability to keep that promise depends on caseloads, building assignments, travel time between sites, bell schedules, and how many providers resigned in August.
